It's what's known as "rounding off". He has 692 transactions with 687 positive (3 neg and 2 neutral). So, divide 687 by 692 and you get a positive feedback percentage of .9927745%, which is rounded off to 100% (A+).

You will find that many, many A+ sellers have some neg feedback.
