I have a S&W 686-3 in 357. It has scope mount milled in top of barrel. What is this? hunter or what?

Not sure, perhaps modified for the scope mount holes.

I can only find one in SCSW. It listed as having as having a 8 3/8" barrel, "broached with scope mount".

1100 made in 1989. Product code #104251. No other information, such as model name, or who they were made for?
