Will a Rem Model 121 forend fit a Mod 12?

I think so but it will look wrong, big beavertail vs the small skinny corncob. I started shooting my 121 in the late 60's, the 12 was small person size and dad preferred it. It pointed down hill for me so the man size 121 was wonderful.

added Years ago I made a rotating tube magazine speed loader. Pull the tube, drop in a full magazine load, then replace the tube.

The screw holes line up and it looks like it should fit. BUT, the 121 forend is longer and will cover up the 12's loading port. You will have to cut off four inches of the 121's forend to be able to load the magazine tube of the 12 with a twenty-one inch barrel. Another way would be to drill out the bottom of the forend over the loading port to insert cartridges thru the forend into the loading port.
