I have no idea how this happened.

First off, I obey all the road laws, I don't drive crazy, I don't do anything stupid, and I treat my truck with care. So I've got no idea how this happened. But I noticed a knocking under the hood last night. Was too dark to see anything, and didn't have a good flashlight, so I just went on. This morning I checked again, and my fan was locked up. So I move it by hand, and notice that one of the blades is bent into a U. Not only that, but all the non-bent blades are now scraping the very bottom.

no cat guts?
Looks relatively clean around where I keep my truck, and I'm pretty sure I saw the neighborhood cat recently.
It's not entirely impossible. Usually you get some fur, blood and and worse case, guts.
Look really really good. It's hard to see all the way back without some effort. Do this because if he is in there and you don't find him, you will notice people not wanting to park next to your truck in a few days.

I can't think of anything that would bend a blade into a U. the rest of the blades aren't going to be scraping the shroud unless the shroud moved. still wouldn't bend a blade into a U. must be gremlins
If you bend one blade, it will throw the others out of balance. Causing them to rub the shroud, sometimes gently not causing damage, and sometimes shearing them completely off.
On older models where the fan bolts directly to the water pump, failed bearings in the pump can cause the fan the wobble enough to hit the shroud.
Once had a WP shaft break and send the fan through the radiator,
Went out the next day expecting to have to replace a belt, and found radiator guts strung all over the engine. [V]
(It was a ferd!)

My first thought, followed by failed motor mounts. Then maybe the transmission mounts, but that should have cocked the front of the motor up unless it's happening during acceleration then maybe it lifts the transmission and drop the front of the motor.
