Bad cartridges?

I went to the range yesterday with a few of my pals and we shot several of our favorite guns. While shooting my Bushmaster 20" AR-type rifle we had 4 cartridges that when fired smoked really badly. Upon closer inspection we discovered that the primer had come completely out of the pocket and was lodged inside the bolt area causign a jam upon feeding of the next cartridge. We examined the remains and it looked like the primers might not have been seated properly.
Has anyone else seen this happen before? I'm thinking about sending the fired cases with loose primers back and asking for a replacement of my entire order (since they won't allow returns) but I want to make sure this isn't being caused by my rifle. By the way, my rifle was meticulously clean and in perfect working order. I ended up shooting another 100 rounds of Black Hills 55Gr FMJ ammo without any problems.
I was using 55Gr NBT "Ultramax" ammo I bought in a 500 lot from Cabelas.

I have had quality issues with some of their handgun ammo. FTF's, split cases and primers falling out. Granted this was a few years ago and I havent bought any since. Couldnt hurt to send back the cases (and primers if you kept them) with a note on what happened. Probably just get more ammo for the trouble though.
I doubt it was the rifle's fault though it would be possible to push a primer out if your firing pin was too strong, (spring tension) but in the same vien I have never heard of an AR having a too strong firing pin.

When guns were invented everything changed. For the first time in the history of the world a frail woman had a chance to sucessfully defend herself and home. My dream is that one of the anti-gun nuts will need a gun for defense and be unable to have one because of their own actions.
