Did none of the other officers know what the truth was? Unless she was the commander of this task force, I don't think she is the problem, just the scapegoat.
Doesn't all add up very well, but then I have found that lots of people want to be in charge. Damn few want to be responsible.

The taxpayers hire these people, the taxpayers pay these people, the taxpayers approve or disapprove of what these people do. The taxpayers should pay attention and act accordingly.

tax payers do no such thing, they elect a sheriff that hires deputies, they elect a mayor that appoints a chief and he does the hiring
I don't know what world you live in, but here we have no such powers to approve anything law enforcement does
There were plenty of opportunities for the Sheriff to get this thing right. Even at the last seconds when the SWAT team was arriving at the house. There were children's toys within feet of the front door. The minivan had child seats, other items for children inside. On the window facing the door the SWAT team breached were those outline stickers of Dad, Mom, Kids.
The Sheriff claims there was no evidence of children present. The family had been there for weeks. The kids played on grass in the front yard.
The Sheriff is either lying his * off or is incompetent. In either case the damage done is criminal and he himself deserves imprisonment.
Too many agencies have swat teams for which there is little need. So, they are tempted to exaggerate the threat. Judges need to be better trained, as they seem to accept a single cop's declaration as fact. They should be requiring a recorded interview with the cop, still photos, & even video of activities at the location. And, they should issue no-knock warrants only if the threat is extreme.
