Blood sugar gone haywire

Had a Dr.'s appointment last Wednesday for A1C test. My blood sugar was 674. I rarely check my blood sugar---have been diabetic for several years and take insulin twice a day. That morning I had a "low blood sugar" attack and drank some OJ with a couple of small donuts before my appointment.
I decided to start checking and keep a record for my Dr. I have another appointment next Wednesday.
Yesterday morning with empty stomach: 1st test--46. Tested again--49. At 1:15pm it was 335 and all I had was a bowl of cornflakes with sliced bananas. At 3:15pm it was 621. At 6:15pm it was 71.
This morning (empty stomach) it was 36 (dangerously low). Tested again--33. At 12:45pm today it was 143.
My Dr. and myself have no clue what's going on.
