There is the chance some crazy fool may have sabotaged the pipe...
My thought also. Other than that, if these "pipeline companies" want the American people on board, they should consider a bit of maintenance. Here is where we could learn something from China. If these "accidents" happen, high mucky mucks in the company should do 15 years hard time, on a chain gang. Bet the accidents would cease. They should also feel lucky, because in China they execute people like that sometimes.

Easy there fella! The CEO isn't the guy to hang. Pipeline crews that work 28 hours every day, finish as fast as possible for that bonus, and are speaking foreign languages would be the last hands on that pipe; and their performance is directly responsible for that pipes' longevity.
The inspectors checking welds, the side-boomer drivers, the welders themselves, and the crews that coated/taped/wrapped it could be blamed also.
There is a possibility that someone dug into, shot at, drilled or hammered into, or intentionally damaged that pipe.
There are already almost as many pipeline rules & regulations as there are gun laws, and both can cause grievous harm in certain cases.
Maintenance is also regulated, regular & roundly supported by the pipeline operators. It's like the advertising budget for most other businesses. It never goes away.
Technology gave us the "smart" pig; an electronically enabled, pressure driven, pipe-sized plug that travels the interior of the pipe, scanning, sampling, tracking, inspecting, & reporting back to the technician.
Pipeline safety is admirable, better than airlines, railroads, highways, or space travel. Except in parts of Africa, The middle east and Asia; something about war-torn $#!? holes and terrorists.
If you knew how many Trillions of gallons weren't spilled in the last eighty years of pipeline use, you'd spit.
