Medical power of Attorney for minor child...

My MIL wants to take the kids from CO to TX for a couple of weeks this summer. I think it is a great idea but I want to send her with a POA for me little ones. I have a couple of questions:

Do I need an attorney for this or is the verbiage I find on the internet going to work?

I see web pages that offer state specific documents for anywhere between $15 and $50. Is this the better route?

I someone has an example they have used please send me some verbiage.

    I or we (parents name)give permission for (your MILs name) to make any medical decisions necessary for the care of kids name. Date it and both of you sign it. Make a seperate one for each kid. Also would not hurt to put the date of last tetnus shot on there too or have a shot record to send with her for each kid.

    Don't know about TX or CO but that is all that is necessary in Maine. We have one for our grandaughter signed by both dad and mom. She stays with us quite a few weekends and a week or so at a time in the summer. Tell the kids to have fun and enjoy the trip.

    working in the er in texas,any family memeber over the age of 18 can give medical attention to a child..if the parents are out of town,,usually we will try call the parents.

    having a piece of paper saying the I give so and so permission to treat my child since they are out of town..works just fine..
