Check to see if the ejector rod is tight.

Is the cylinder release latch screw tight? How about the ejector rod? Have you applied some lube to the ejector rod latch under the barrel up by the muzzle? With the cylinder closed, work the cylinder release latch and watch for free movement of the ejector rod.

When checking the ejector rod remember it is reverse threaded. May also want to check and see if any grease or debris has gotten into the slot in the inside of the frame that the shaft slides in. If so a little brake cleaner will disolve it - must relube after as this leaves it DRY.

As has been said, the ejector rod loosening can be the cause of this and it's reverse threaded.

To tighten it by hand, first load six empty cases to avoid damaging the extractor and it's alignment pins. Now simply hold the rod still and rotate the cylinder in the direction it turns when firing.

As you do this, take care not to tweak the rod. In fact, it being slightly bent could be your problem to start with.
