PSL extraction problem

I have a PSL and when I fire it the empty casing extracts only to the 90% angle from the center line of the barrel before the bolt closes thus "pinching" the casing just about in its center. Happens every shot so I've never been able to fire more than one round at a time before I have to open the bolt and remove the pinched casing.

What would cause this? Old grease in the bolt assembly?, something wrong with the extractor? It never fails to extract the empty but it won't throw it out before the bolt closes on it.
