Log sheet in Excel format

I've searched, but I can't find a good log sheet in excel format. Anybody have one before I go create my own?
Also, if I create my own, what should I include? (I don't have a chrono so I can't put the fps.)
I was thinking:
Caliber
Date
Bullet type
Bullet weight
Powder type
Powder weight
Primer type
Number of loads in the batch
Anything else?
